That is a fourfold difference—a range wide enough to encompass very different clinical realities. Yet many patients are initiated at a standard dose and remain there indefinitely, either because adjustments were never discussed or because neither party in the clinical relationship had enough information to initiate the conversation.

Distinguishing Side Effects From Inadequate Dosing
